The Global State of Harm Reduction 2024 report maps the global harm reduction response, covering a wide range of health and social services for people who use drugs. This report highlights key developments in civil society, drug policy, and funding, and is a collaborative effort involving almost 300 harm reduction practitioners, academics, advocates, and activists from 101 countries. It provides an independent analysis of the state of harm reduction globally.

As you may know, northern Thailand has been severely impacted by massive flooding, with Chiang Mai experiencing the worst flood in 50 years. Thousands of households were submerged, and as the waters recede, many are now grappling with the overwhelming aftermath. The effects on poor and marginalized communities have been devastating, with significant loss of livelihoods.
This crisis serves as another stark reminder of the growing impact of climate change. Unfortunately, the Southeast Asia Harm Reduction Association (AHRA) premises were also flooded, with waters reaching 1.5 meters. We have begun the cleanup process and are incredibly thankful for the support and assistance we’ve received to help us restore operations.

We are thrilled to share a significant milestone in our efforts to combat drug use problem in Myanmar.
The CPI AIS (https://lnkd.in/grZ3WpUX) team, in collaboration with our esteemed partners AHRN and MAM, has initiated the introduction of Buprenorphine (BPN) to facilitate Opioid Substitution Therapy (OST) for Myanmar’s estimated 93,000 drug users.
BPN provision has been launched in both Myitkyina and Yangon led by the National Drug Dependency Treatment and Research Unit. Last month, we administered BPN to three new clients, and the results have been nothing short of remarkable. These individuals have experienced improved treatment outcomes and greater stability in their conditions, which is a testament to the effectiveness of BPN as an alternative treatment option.
Buprenorphine’s unique biological effects, characterized by partial agonism, make it a valuable addition to our OST program. It offers hope and a pathway to recovery for those struggling with opioid addiction in Myanmar.

The VNGOC in cooperation with UNODC will hold an online workshop on 7 September 2023, 3 PM to 4.30 PM CEST (Vienna time).
In preparation for the 2024 Mid-Term Review of the Commission on Narcotic Drugs (CND) the Commission will conduct a comprehensive stock-taking of progress made in implementing all international drug policy commitments to address the challenges identified in the 2019 Ministerial Declaration.
To better facilitate civil society engagement in the first thematic discussion the workshop will give room to discuss the October topics and will give guidance on the speaker selection process.

The Guide: Words Matter! emphasizes the importance of using respectful and empowering language when referring to Words-Matter-Language-Guide It advocates for people-first language, avoiding stigmatizing terms, and promoting understanding and inclusivity. The guide provides alternative terminology and encourages individuals to be thoughtful about their language choices. It also highlights the impact of language on stigma, discrimination, and access to healthcare services. The guide was developed by the International Network of People Who Use Drugs (INPUD) and the Network of Asian People who use Drugs (NAPUD) in consultation with various regional networks. It is a valuable resource for promoting dignity, respect, and understanding for individuals who use drugs.

The study examines harm reduction for drug use in Vietnam, focusing on perspectives of drug users, healthcare experts, and policymakers. It found that drug users see methadone treatment as a step toward quitting, while policymakers prefer punitive measures. Current programs lack specialization, and there’s a lack of info for policymakers on effective harm reduction. The study calls for a more humane harm reduction approach in Vietnam and similar countries. Published in March 2022.
Conclusions. Harm reduction principles have not been fully adopted in Vietnam. A harm reduction strategy based on a more humanistic approach that goes beyond a bio medicalized approach is urgently needed in Vietnam and other countries in the Global South.

The study discusses the shift in harm reduction programs from focusing on opioid users to addressing methamphetamine users (PWUM) in Jakarta, Indonesia. While harm reduction efforts have largely centered around opioid injection, the rising prevalence of methamphetamine use has led to underserved PWUM populations. This study examines a newly implemented outreach program for methamphetamine users and identifies key lessons from the transition. The study employed various data collection methods, including documents, interviews, questionnaires, and observations. Four main areas of focus emerged: 1) Establishing connections and building trust with different types of PWUM; 2) Adapting safer smoking kits for local contexts; 3) Adjusting partnerships to address mental health concerns; and 4) Navigating interactions with local law enforcement. The study emphasizes the importance of involving PWUM in program development, understanding their broader needs, and promoting harm reduction beyond specific drug use methods. Additionally, securing funding for harm reduction among non-opioid users is crucial for program sustainability.

The study compared the effectiveness of HIV clinic-based buprenorphine plus naloxone treatment with referral for methadone maintenance therapy in achieving HIV viral suppression among people with HIV and opioid use disorder in Vietnam. The trial involved 281 participants across six HIV clinics. While both treatment methods improved viral suppression rates at 12 months, buprenorphine plus naloxone did not prove non-inferior to methadone maintenance therapy. Retention on medication was lower for buprenorphine, and participants on buprenorphine more often experienced serious adverse events and deaths. The study suggests that while integrated buprenorphine and HIV care could enhance opioid use disorder treatment access, support for buprenorphine adherence is crucial for improving HIV outcomes. The Global State of Harm Reduction report from 2020 to 2022 highlighted significant shifts and challenges in global harm reduction efforts. During this period, harm reduction initiatives gained traction worldwide. New needle and syringe programs were established in African countries, and officially sanctioned drug consumption rooms, including one in Mexico, provided safe spaces for supervised drug use. Essential services like opioid agonist therapy and needle exchange programs expanded, while the availability of take-home naloxone and peer-driven distribution models increased, benefiting users in multiple countries.
Policy advancements were evident, with over 100 countries incorporating harm reduction references into their national policies. However, persistent challenges emerged, especially within Southeast Asia. The COVID-19 pandemic disrupted harm reduction programs in Asian countries, causing closures due to government restrictions, funding shortages, and supply chain disruptions. While peer-led initiatives addressed gaps, marginalized groups faced limited access to harm reduction services.
Hepatitis C testing and treatment efforts showcased successes. Community-driven models like C-FREE in Thailand demonstrated the efficacy of community-based approaches. In Vietnam, integrated care for at-risk populations expanded. These achievements emphasized the significance of community involvement.
Nonetheless, challenges persisted, necessitating increased funding and sustained efforts to ensure equitable access to harm reduction and healthcare services, especially in the face of ongoing disruptions from the pandemic.
